Mr. Thomas Hayhurst is a senior and a person with a disability. On 7-31-2015, an Access to Independence Independent Living Coordinator made a visit to do an intake with Thomas who wanted to gain his independence back. The IL Coordinator went over services and Thomas immediately agreed to work with us. Thomas had been hospitalized for over month and had almost lost his life due to an illness. While fighting for his life he lost everything he had and became homeless.
Thomas said,” Whatever it takes I want to live on my own again and I will do whatever it takes to do so.” Thomas fought for his health and won. He was approved to leave the skill nursing facility and with the help of an IL Coordinator planned his transition out of the nursing facility. The IL coordinator and Thomas went over the things he would need to make his transition successful. First, he needed to find housing, buy furniture, clothing, household items, and groceries. Thomas is on a fixed income and did not have enough money to buy all of the things necessary to make his transition successful so the IL Coordinator applied for the Department of Rehabilitation funding and Thomas was approved!
The IL Coordinator helped to pick up and deliver the rental application to the low-income senior apartment building. After a couple of months, Thomas was approved for his new apartment!
Thomas said, “It feels so good to be out in my own apartment paying my own bills and being a part of the community, I am so happy that Access to Independence helped me with the transition program.”
