Homeward Bound Transition Initiative
The Homeward Bound Transition Initiative is a person-centered program that supports individuals transitioning from institutional or skilled nursing settings into the community. The program helps address barriers that can make returning home difficult, including the need for accessibility modifications and in-home supports.
As Access to Independence celebrates 50 years of service, this video highlights one example of Homeward Bound in action by showing how an accessible bathroom remodel supported a consumer’s ability to live safely and independently at home. Each Homeward Bound transition is guided by the individual’s goals, preferences, and needs, with supports tailored to promote independence, dignity, and community living.

Homeward Bound Initiative Services
Safe Transition Assessment
We aim to facilitate safe and permanent transitions from institutional settings to home. The first step is to conduct a safe transition assessment. Our transition coordinator will meet you at the facility to conduct this assessment along with your support team, including discharge planners, your medical team, family, or occupational therapists. The assessment tool helps us identify your needs, from housing to basic needs and social support, to ensure a safe and permanent transition. From the assessment, we create an individualized plan that is tailored to your unique needs. The plan outlines every step we will take and the timeline in which each step will occur.

We provide a range of home modifications, including:
Project Coordination
We help coordinate the entire modification process, from design to construction, ensuring that everything is completed to the highest standards of safety and accessibility.
Assistive Technology Integration
Our team can recommend and install assistive devices to enhance your home’s functionality, such as automated door openers, voice-activated controls, and mobility aids.
