Community Needs Assessment - All Branches
We want to identify some of the challenges that people with disabilities face in the cities they live in. In particular, we would like to identify barriers in accessing services and any gaps in our current services provides. This survey is anonymous, but we will be using the summarized information we collect to improve services in the local area.
Community Needs Assessment 2023 - Hawaii
The survey found at the link below is from the Hawaii State Independent Living Council (SILC). The Community Needs Assessment is used to gather the community's input on independent living services. We ask that our Hawaii consumers and soon-to-be consumers complete this survey.
